Abstract

In Burkina Faso, malaria is a major public health problem and the leading cause of medical consultations and hospitalizations. Dihydroartemisinine-piperaquine (DHA-PPQ) is used as a first-line treatment for uncomplicated malaria. Ex vivo susceptibility testing is one of the best methods for assessing the efficacy of antimalarial compounds. The aim of this study was to test the ex vivo susceptibility of clinical isolates of Plasmodium falciparum to DHA and PPQ in the city of Bobo-Dioulasso.

A cross-sectional study was conducted at three health centers between June 2024 and January 2025. Blood samples were collected from subjects with malaria following confirmation by thick smear/blood smear, with a parasite density greater than 1%. Ex vivo sensitivity was measured after culturing the parasites, and 50% inhibitory concentrations (IC50) were determined using a standard 72-hour growth inhibition assay.

A total of 133 patients were included in the study. The growth inhibition assay for isolates had a success rate of 90.98% (121/133) for DHA and 85.71% (114/133) for PPQ. Analysis of the IC50 values revealed median values of 1.88 nM (IQR: 0.31–9.58) and 8.49 nM (IQR: 1.43–39.67) for DHA and PPQ, respectively.

The study demonstrated good sensitivity of the tested clinical isolates. Surveillance systems must be strengthened to enable earlier detection of signs of resistance.
