Introduction: Sexual and reproductive health (SRH) remains a major determinant of the well-being of women and families in Burkina Faso. Family planning (FP) services have been free since 2020. The purpose of this article is to estimate the households costs of FP services, post-abortion care (PAC) and legally safe abortion care (SAC) .
Methodology: A cross-sectional intervention-control study on the household costs of FP, PAC and legally SAC was conducted among service clients in December 2022. A descriptive analysis of median direct (medical and non-medical) and indirect costs were performed. Costs were in African Francophone Community (F CFA) francs.
Results: The median total household costs in the intervention and control facilities for FP were 300 F CFA (88%) and 216 F CFA (99.5%); PAC costs were 6,163 CFA francs (32%) and 1,530 CFA francs (88%); and legally SAC costs were 57,922 CFA francs (99%). The median costs of FP and PAC services were not significantly different for the intervention and control FS. Sixteen per cent of FP clients paid 650 CFA francs for the direct medical costs, mainly for contraceptive products and consumables due to stock shortages.
Conclusion: Access to SRH services remains hampered by the persistence of direct payments by households. Strengthening public funding and effectively integrating free SRH services are essential levers for achieving universal health coverage.
